Family Liason Officer

Hi, i would like to introduce myself, my name is Sammi Zafir and I am the Family Liaison Officer at Fairfield School.

My role involves:

• Providing information about services available for children with disabilities and for their families.
• Assessing a child and family's changing needs which will lead to individual care plans.
• Providing culturally sensitive support, as well as advice, guidance and advocacy if required.
• Liasing with professionals in order to co-ordinate services and share information.
• Identify problems and concerns and help improve methods of communciation between all parties involved.
• Empower families to mangage their long term care plans and to independently access mainstream services and welfare benefits that they are entitled to.
• Work alonside Social Services - Young Peoples Activity Team during the school holidays and help with the running of the Play schemes.
• Co-ordinate and facilitate a Parents support group at Fairfields School.
• Develop partnerships between parents, school and professionals involved.
• Day surgery at Fairfields, where parents and carersare able to come and highlight ongoing concerns or issues that have arisen and are seeking guidance and support.
• Parents/carers and children accessing a multi sensory toy library at Fairfields.
